Rosacea in Crohn's Disease: Effect of Rifaximin.

چکیده
Leonard B. Weinstock, MD Washington University School of Medicine, St Louis, MO; Specialists in Gastroenterology, LLC, St Louis, MO Rosacea is a skin disease that manifests as flushing, persistent skin redness (ie, erythematous form), small rounded bumps on the skin (ie, papulopustular form), or dilated superficial blood vessels (ie, telangiectasia). The pathophysiology of rosacea is unknown, but alterations in the immune system, either by organic disease (eg, inflammatory bowel disease [IBD]) or immune-altering medications, may predispose individuals to the development of rosacea. Standard therapy for rosacea includes topical therapies (eg, sodium sulfacetamide) and oral antibiotic medications (eg, doxycycline, tetracycline, minocycline, erythromycin, and metronidazole) for patients who are intolerant of topical therapies.

Ocular Rosacea: Treatment with Rifaximin, a Non-Systemic Antibiotic
Background: Rosacea is associated with Crohn’s disease, liver disease, chronic pancreatitis, achlorhydria, Helicobacter pylori and recently with idiopathic small intestinal bacterial overgrowth (SIBO). Two publications demonstrated that rifaximin, a non-absorbed, gut-directed antibiotic for SIBO led to improvement in facial rosacea. Ocular manifestations occur in up to 58% of rosacea patients a...
متن کاملComment on: Rifaximin modulates the colonic microbiota of patients with Crohn's disease: an in vitro approach using a continuous culture colonic model system.
OBJECTIVES Rifaximin, a rifamycin derivative, has been reported to induce clinical remission of active Crohn's disease (CD), a chronic inflammatory bowel disorder. In order to understand how rifaximin affects the colonic microbiota and its metabolism, an in vitro human colonic model system was used in this study. METHODS We investigated the impact of the administration of 1800 mg/day of rifax...
متن کاملTreatment of Refractory Crohn's Disease and Pyoderma Gangrenosum with a Combination Regimen of Rifaximin, Gentamicin and Metronidazole
The etiology of Crohn's disease (CD) remains controversial. It is hypothesized that CD is the result of an abnormal immune response to the gut flora in genetically susceptible hosts. However, an infectious etiology has not been completely ruled out. Antibiotics have been utilized with some success to modify the course of the disease. Here, we report a patient with CD and pyoderma gangrenosum re...
متن کاملThe Possible Combined Action of Different Trigger Agents in Rosacea.
We read with great interest the paper entitled ‘‘Oral ivermectin to treat papulopustular rosacea in a immunocompetent patient.’’ by Hernández-Martín1 that prompted us to make some observations and to report our experience. The Author described a patient with papulopustular rosacea that has been considered immunocompetent but it was not specified whether laboratory examinations have been perform...
متن کاملTherapeutic role of rifaximin in inflammatory bowel disease: clinical implication of human pregnane X receptor activation.
Human pregnane X receptor (PXR) has been implicated in the pathogenesis of inflammatory bowel disease (IBD). Rifaximin, a human PXR activator, is in clinical trials for treatment of IBD and has demonstrated efficacy in Crohn's disease and active ulcerative colitis. In the current study, the protective and therapeutic role of rifaximin in IBD and its respective mechanism were investigated. PXR-h...
